Does your child need a consult?
- Most PPO's do not require a referral, HMO's and
certain private Insurances require a referral by
your pediatrician or family physician
- In any case we will arrange the earliest possible
appointment , referring physicians may contact us
if there is a need for same day consultation.
- We encourage bringing along all supportive
information: laboratory tests, X-ray results, list of
medications, etc.
- You may fill demographic and medical history
forms available in Resources page and bring
them to appointment
